About the Role

We are seeking a motivated and compassionate Physical Therapist (PT) to provide high-quality home care services. This role offers the unique opportunity to build and manage your own caseload while maintaining flexibility and autonomy in your schedule.

Responsibilities

  • Provide in-home physical therapy evaluations and treatments
  • Develop individualized plans of care based on patient needs and goals
  • Manage and grow your own caseload within our service area
  • Accurately document visits, progress notes, and discharge summaries
  • Communicate effectively with patients, families, and healthcare team members
  • Ensure compliance with all state, federal, and agency regulations

Qualifications

  • Licensed Physical Therapist (PT) in the New York state
  • Strong clinical and interpersonal skills
  • Ability to work independently and manage time effectively
  • Home care experience preferred but not required
  • Reliable transportation and willingness to travel locally
